New PatientsWe warmly welcome new patients to Rotem Medical Family Practice. To make your first visit as smooth as possible, please:

• Book online via AutoMed or call our reception team to arrange your first appointment.
• Arrive 10-15 minutes early to complete a new patient registration form or complete the form online prior to the appointment.
• Bring the following:

A valid Medicare card (if eligible)

• Concession, Pension or DVA card (if applicable)
• Any current medications or a medical list
• Relevant referrals, test results, or hospital discharge summaries
• For children, their Blue Book (NSW Health Record Book)
